Welcome to Sunflower Path Counseling. By accessing or using our website and telehealth counseling services, you agree to the following Terms and Conditions. Please read these carefully.
1. Practice Information
• Practice Name: Sunflower Path Counseling
• Therapist: Monica Alvarez, LCPC
• Licensure: Licensed Clinical Professional Counselor (LCPC) in Illinois and California.
• Services Provided: Individual counseling, adolescent counseling (ages 12+), trauma-focused therapy (including EMDR), and psychological evaluations (including immigration-related evaluations). Services are provided via telehealth only.
2. Eligibility
• Services are available only to residents of Illinois and California where Monica Alvarez holds an active license.
• By using our services, you confirm that you are located in one of these states at the time of service.
• Clients must be at least 12 years old. For minors, a parent or legal guardian must provide consent.
3. Telehealth Services
• Counseling is conducted securely via HIPAA-compliant telehealth platforms.
• You acknowledge that telehealth has limitations compared to in-person counseling, including technology disruptions, risks to confidentiality, and limited emergency response options.
• You are responsible for ensuring privacy on your end during sessions.
4. Emergencies
• Sunflower Path Counseling does not provide emergency or crisis services.
• If you are experiencing a crisis or emergency, call 911, go to your nearest emergency room, or call the 988 Suicide & Crisis Lifeline.
5. Confidentiality
• Your privacy is protected under state and federal law (HIPAA).
• Exceptions to confidentiality include situations where there is a risk of harm to yourself or others, suspected abuse/neglect of a minor, elder, or dependent adult, or as otherwise required by law.
6. Payment & Fees
• Fees for counseling sessions and evaluations are provided prior to scheduling.
• Payment is due at the time of service unless otherwise agreed.
• Sunflower Path Counseling may accept private pay and/or selected insurance (details provided separately).
• Missed appointments or cancellations without 24-hour notice may result in a cancellation fee.
7. No Guarantee of Results
• Counseling is a collaborative process but results cannot be guaranteed.
• Progress depends on individual commitment, circumstances, and other factors outside of the therapist’s control.
8. Technology & Website Use
• Our website provides information about services and resources but does not replace professional mental health care.
• You agree not to misuse, copy, or distribute website content without prior written consent.
• We make no warranties that the website or telehealth platforms will always be available, secure, or error-free.
9. Limitation of Liability
• Sunflower Path Counseling and Monica Alvarez, LCPC, are not liable for any direct, indirect, incidental, or consequential damages resulting from the use or inability to use services, including technical failures, data breaches, or reliance on website information.
10. Informed Consent
• Before beginning counseling, clients will review and sign an Informed Consent for Treatment form that outlines services, risks, benefits, and rights.
• By using this website or scheduling a session, you acknowledge understanding of these terms.
11. External Links & Resources
• The website may include links to external sites. Sunflower Path Counseling is not responsible for the accuracy, reliability, or content of third-party websites.
12. Modification of Terms
• We may update these Terms & Conditions at any time. Updates will be posted on this page with a revised effective date.
• Continued use of our website and services after changes constitutes acceptance of the revised terms.
13. Governing Law
• These Terms & Conditions are governed by the laws of the State of Illinois and the State of California.
